Short-Term Respite Care provides one to 30 nights of loving, safe,
medically supervised care for the senior adult who cannot live alone
and whose caregiver need some time apart for any reason. We encourage short-term visitors to participate in the full range of programming, services
and activities offered at the Nursing Center. Reservations are
required.
Requirements for Admission
Four to six weeks prior to admission, a Respite visitor
or his or her caregiver needs to complete an application and a medical
information form.
Visitors will need to provide copies of:
Medicare card
Medicaid card
Other insurance cards, if any
Social Security card
Advanced Directives
(Living Will, Health Care Proxy, Durable Power of Attorney, etc.)
Respite applicants should contact a visiting nurse association,
the Department of Social Services in the county in which they live, the New
York State Department of Health or a certified homecare agency to perform an evaluation to
ensure that Daughters of Sarah
can meet the individuals healthcare needs.
