Among the Daughters



In Hospice at Daughters of Sarah, uniquely trained nursing center and Communinity Hospice staff combine their sensitive skills and caring to provide culturally sensitive end of life care. Especially valuable for Jewish families is the ease and thoroughness of attention to Jewish preferences, traditions, and nuances at this precious time of life in our Jewish nursing home.

Hospice at Daughters of Sarah staff supports and assists terminally ill individuals and their loved ones to cope with the challenges of life's ending. Community Hospice collaborates with Daughters of Sarah staff in the Nursing Center to enhance quality of life for residents entrusted to our care.

Philosophy

Life-affirming services are provided by an interdisciplinary team under the direction of a primary care physician. Our goal is to ensure comfort and dignity in the warm and loving presence of family, friends and caregivers.

Hospice at Daughters of Sarah provides symptom-directed, supportive care and addresses the physical, emotional, social and spiritual needs of residents, families and caregivers.

Referrals and Eligibility

Hospice at Daughters of Sarah accepts referrals from any individual. In all cases we will contact the primary care physician to continue the admissions process.

The following criteria are factors in determining eligibility:

• Whether illness is in its final stages

• Appropriateness of supportive, comfort-oriented care

• Report of primary care physician

• Residence of individual in service area

Fees

Services are covered by Medicare, Medicaid and many private insurance companies and HMOs. No one is refused care because of inability to pay. Hospice assists with preparing paperwork to bill and obtain coverage for any service it provides.
